Vitamin D and Cognition: Deficiency Is Common, Supplementation Trials Are Disappointing

Low vitamin D has been associated with worse cognitive outcomes in observational research. Randomised trials of supplementation have largely failed to show cognitive benefit.

Vitamin D follows a pattern that recurs throughout nutrition research on cognition, and recognising the pattern is more useful than the specific findings.

The observational picture

Low vitamin D status has been associated in multiple observational studies with worse cognitive performance and higher rates of cognitive decline and dementia diagnosis. The associations have been reasonably consistent.

There is biological plausibility. Vitamin D receptors are present in brain tissue, and the vitamin is involved in processes including calcium regulation and immune function that are relevant to neural health.

The trial picture

Randomised trials of vitamin D supplementation for cognitive outcomes have generally not demonstrated meaningful benefit. Large trials examining supplementation across a range of outcomes have not produced convincing cognitive results.

The recurring pattern

Observational association, plausible mechanism, disappointing trials. This has happened with vitamin E, with B vitamins in non-deficient populations, with omega-3, and now with vitamin D. When a pattern repeats this consistently across nutrients, it is telling you something about the research design rather than about each nutrient.

Why the pattern occurs

Several explanations apply here as elsewhere:

  • Confounding. Low vitamin D is associated with being indoors, being less physically active, being frail, being unwell, obesity and lower socioeconomic status. Each has its own relationship with cognition.
  • Reverse causation. People in early cognitive decline go outside less, which lowers vitamin D. The low level may be a consequence rather than a cause.
  • Baseline adequacy. Supplementing people who are not deficient would not be expected to help.
  • Timing. If status over decades matters, a trial running a few years in older adults may be testing the wrong window.

What vitamin D is genuinely for

Deficiency causes real and serious problems — rickets in children, osteomalacia in adults, and contributions to bone loss and fracture risk. Vitamin D's role in bone health and calcium absorption is well established.

For older adults, this matters cognitively through an indirect route: falls and fractures. A fall causing head injury is a genuine cognitive risk, and bone and muscle health are part of preventing that.

Who is at risk of deficiency

  • People with limited sun exposure, including those who are housebound or in residential care
  • People living at higher latitudes, particularly in winter months
  • People with darker skin, who synthesise less vitamin D from a given sun exposure
  • Older adults, in whom skin synthesis becomes less efficient
  • People who cover most of their skin
  • People with conditions affecting fat absorption
  • People with obesity, in whom vitamin D distribution differs

Public health bodies in a number of countries recommend supplementation for some or all of the population during winter months, and recommendations vary by country. Following local guidance is more sensible than following a figure from an article.

Practical position

Vitamin D supplementation is reasonable for bone health where local guidance recommends it or where deficiency is identified. Testing is available where there is clinical reason.

Supplementing specifically to improve memory is not supported by the trial evidence. High-dose supplementation is not benign — excessive intake can cause hypercalcaemia — and doses well above recommended intake should involve a doctor rather than self-direction.
