Thyroid Function and Memory: A Routine Test Worth Asking About

Thyroid dysfunction can produce cognitive symptoms, is common, and is identified by a standard blood test. It is one of the simplest reversible causes to rule out.

Thyroid testing appears in most standard evaluations of memory complaints for a straightforward reason: thyroid dysfunction can produce cognitive symptoms, it is reasonably common, and the test is inexpensive and routine.

What the thyroid does

The thyroid produces hormones that regulate metabolic rate across essentially every tissue, including the brain. When output is too low or too high, systems throughout the body are affected.

Hypothyroidism

An underactive thyroid is the more common problem, particularly in older adults and in women. Symptoms can include:

  • Slowed thinking, difficulty concentrating and memory complaints
  • Fatigue and sleepiness
  • Low mood, which can itself produce cognitive symptoms
  • Cold intolerance
  • Weight gain
  • Constipation
  • Dry skin and hair changes

Severe untreated hypothyroidism can produce pronounced cognitive impairment. Milder cases produce subtler symptoms that are easy to attribute to ageing, stress or overwork.

Hyperthyroidism

An overactive thyroid is less common but also relevant. It can produce anxiety, restlessness, difficulty concentrating, irritability, tremor, palpitations, heat intolerance, weight loss and sleep disturbance. Concentration problems here often stem from anxiety and poor sleep rather than from slowing.

In older adults, hyperthyroidism can present atypically — with apathy and low mood rather than the classic agitated picture — which makes it easy to miss.

Why symptoms get attributed elsewhere

Fatigue, low mood, poor concentration and weight change are attributable to a dozen ordinary causes. Thyroid dysfunction is identified by testing, not by recognising a distinctive symptom pattern.

Testing

Thyroid function is assessed by blood test, usually starting with TSH — thyroid stimulating hormone — with additional measures added depending on the result. It is part of the standard workup for cognitive complaints, alongside vitamin B12 and other measures.

If you are being evaluated for memory concerns and thyroid function has not been checked, it is reasonable to ask about it.

Subclinical dysfunction

A frequent finding is subclinical hypothyroidism, where TSH is elevated but thyroid hormone levels remain within range. Whether this warrants treatment is genuinely debated, and depends on the degree of elevation, symptoms, age, antibody status and other factors.

Evidence on whether treating subclinical hypothyroidism improves cognitive outcomes is mixed, with some trials in older adults finding no clear benefit on symptoms. This is a clinical judgement rather than something to press for or refuse based on general reading.

Treatment and cognition

Overt hypothyroidism is treated with hormone replacement, and cognitive symptoms attributable to it commonly improve with adequate treatment. Hyperthyroidism has several treatment approaches depending on cause.

Two practical points for people already on thyroid medication. Dose requirements change over time, so periodic monitoring matters — symptoms returning may reflect a dose that needs adjusting. And several medications and supplements, including some calcium and iron preparations, interfere with absorption of thyroid hormone replacement, which is worth checking with a pharmacist.

Keeping it in proportion

Thyroid dysfunction is one of several reversible contributors, not a common explanation for most memory complaints. Its value is that it is cheap to rule out and consequential if missed — which is exactly why it belongs in a standard evaluation.
